Health Connections Inc. (HCI) is a Milwaukee-based, 501(c)(3) nonprofit community health organization providing accessible, culturally responsive healthcare and public health services to underserved populations. Founded and led by a native of Milwaukee, HCI is deeply rooted in the communities it serves and is committed to advancing health equity through both clinical care and community-based prevention.
HCI operates a community clinic and delivers services across Milwaukee through health screenings, education, outreach, and partnerships with trusted community organizations. Our work spans primary and preventive care, chronic disease prevention (including heart health and cancer screening), vaccination efforts, and community health worker–led engagement.
At HCI, we believe health is built through trust, relationships, and meeting people where they are. Our team works across clinic and community settings to reduce barriers to care and create pathways to healthier lives.
