About Health Connections Inc. (HCI)

Health Connections Inc. (HCI) is a Milwaukee-based, 501(c)(3) nonprofit community health organization rooted in trust, relationships, and lived experience. We partner with community members, churches, grassroots organizations, and local leaders to bring health education, screenings, and navigation directly into neighborhoods.

Registered Nurses at HCI practice at the intersection of clinical care and community health. Our RNs provide direct patient care in the clinic while also leading screenings, vaccinations, education, and outreach at community sites. This role is ideal for nurses seeking meaningful, relationship-centered work without hospital-based burnout.

Position Summary

The Registered Nurse (RN) provides high-quality, patient-centered care in the Health Connections Inc. clinic and at community-based sites across Milwaukee. The RN supports preventive services, chronic disease management, screenings, vaccinations, and care coordination while working closely with Medical Assistants, Community Health Workers, and HCI’s Public Health staff.

Compensation

$34–$38 per hour, commensurate with experience and funding source

Essential Duties & Responsibilities

Clinical Responsibilities (In-Clinic)

• Perform patient assessments, triage, and vital signs
• Administer vaccines and medications per protocol
• Support preventive screenings and chronic disease management
• Assist providers with patient flow, documentation, and care coordination • Complete accurate EMR documentation and follow-up tasks
• Support lab workflows, point-of-care testing, and result communication • Maintain infection control and HIPAA/OSHA compliance

Community-Based Responsibilities

• Provide nursing services at community events, health fairs, churches, schools, and outreach sites
• Conduct blood pressure checks, glucose screenings, cholesterol POCT, and vaccination clinics

• Deliver culturally responsive health education
• Identify individuals needing follow-up care or navigation support
• Collaborate with CHWs, MAs, volunteers, and community partners • Document community encounters and screening results accurately

Care Coordination & Follow-Up

• Communicate screening results to patients
• Support referrals, appointment scheduling, and follow-up care
• Assist with navigation for cancer screenings, primary care, and specialty services • Participate in quality improvement activities

Education & Experience

• Active Wisconsin Registered Nurse (RN) license required
• Minimum 2 years of nursing experience required
• Outpatient, community health, or public health experience preferred • CPR/BLS certification required

Physical & Work Environment

• Ability to stand and walk for extended periods
• Ability to lift up to 40 pounds
• Ability to work indoors and outdoors at community sites • Fast-paced clinic and community environments
