  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions
  • Develop and oversee complex treatment plans
  • Lead advanced illness, goals-of-care, and advance care planning discussions
  • Collaborate with PCPs, caregivers, and interdisciplinary team members
  • Provide patient and caregiver education
  • Maintain accurate clinical documentation
Ideal CandidateThis opportunity is ideal for Nurse Practitioners and Physician Assistants seeking greater schedule flexibility, clinical independence, and meaningful patient relationships. You'll care for patients with complex medical needs while enjoying the autonomy to manage your schedule and focus on delivering personalized, value-based care. Required Qualifications Nurse Practitioners
  • Graduate of an accredited MSN or DNP program
  • Active and unrestricted Nurse Practitioner license, or ability to obtain by start date
  • Board certification through AANP or ANCC in an approved specialty
Physician Assistants
  • Graduate of an accredited Physician Assistant program
  • Active and unrestricted Physician Assistant license, or ability to obtain by start date
  • NCCPA certification
Additional Requirements
  • DEA licensure/prescriptive authority or ability to obtain post-hire
  • Ability to obtain a collaborative practice agreement, if applicable
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of a field-based role
Preferred Qualifications
  • 1+ years of clinical experience, preferably in long-term care
  • Experience caring for patients with complex behavioral, social, or functional needs
  • Knowledge of geriatrics, chronic illness, advanced illness, and end-of-life care
  • Proficiency with EMR systems and clinical documentation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
