Advanced Practice Clinician (NP/PA) - Senior Community CareOptum | Multnomah & Clackamas Counties, OR | Full-Time | Field-Based Join Optum's Senior Community Care team and make a meaningful impact caring for medically complex patients in community-based residential settings, including assisted living, independent living, adult foster homes, senior living communities, and private homes.This role is ideal for Nurse Practitioners and Physician Assistants who are energized by complex clinical decision-making and relationship-based care. As the primary Advanced Practice Clinician for your patient panel, you will manage patients with multiple chronic conditions, advanced illnesses, functional limitations, and complex treatment needs, all while being supported by a fully integrated interdisciplinary tea.Why Providers Love This Role

Make an Impact with Complex Patient Care
Care for some of Optum's highest-risk Medicare members, including patients with:
  • Multiple chronic conditions and co-morbidities
  • Advanced disease states
  • Complex treatment plans
  • Behavioral, functional, and social care needs
  • Advanced illness and end-of-life care considerations
You'll assess the whole patient, partnering with caregivers, facility teams, PCPs, and other healthcare professionals to improve outcomes, reduce unnecessary hospitalizations, and enhance quality of life.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions, including diagnostics and interventions
  • Develop and oversee complex, individualized treatment plans
  • Lead advanced illness, goals-of-care, and advance care planning discussions
  • Collaborate with interdisciplinary team members, caregivers, and community partners
  • Provide patient and caregiver education
  • Maintain accurate clinical documentation
Ideal Candidate

Required Qualifications
Nurse Practitioners
  • Graduate of an accredited MSN or DNP program
  • Active and unrestricted Nurse Practitioner license or ability to obtain by start date
  • AANP or ANCC certification in an approved specialty
Physician Assistants
  • Graduate of an accredited Physician Assistant program
  • Active and unrestricted PA license or ability to obtain by start date
  • NCCPA certification
Additional Requirements
  • DEA licensure/prescriptive authority or ability to obtain
  • Ability to obtain a collaborative practice agreement, if applicable
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of a field-based role
Preferred Qualifications
  • 1+ years of clinical experience, preferably in long-term care
  • Experience caring for patients with complex behavioral, social, and functional needs
  • Knowledge of geriatrics, chronic illness, advanced illness, and end-of-life care
  • Proficiency with EMR systems and clinical documentation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
