Join Optum's Senior Community Care team and make a meaningful impact caring for medically complex patients in community-based residential settings, including assisted living, independent living, adult foster homes, senior living communities, and private homes.This role is ideal for Nurse Practitioners and Physician Assistants who are energized by complex clinical decision-making and relationship-based care. As the primary Advanced Practice Clinician for your patient panel, you will manage patients with multiple chronic conditions, advanced illnesses, functional limitations, and complex treatment needs, all while being supported by a fully integrated interdisciplinary tea.Why Providers Love This Role

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
