Advanced Practice Clinician (NP/PA) - Senior Community Care
Optum | Multnomah & Clackamas Counties, OR | Full-Time | Field-Based
Join Optum's Senior Community Care team and practice at the top of your license while providing longitudinal care to medically complex patients in community-based residential settings, including assisted living, independent living, adult foster homes, senior living communities, and private homes.
As the primary Advanced Practice Clinician for your patient panel, you will have the autonomy to make clinical decisions, manage your own schedule, and drive patient outcomes, backed by a fully integrated interdisciplinary team. Our value-based care model prioritizes quality over volume, with no productivity quotas or RVU requirements.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions
  • Develop and oversee complex treatment plans
  • Lead advanced illness, goals-of-care, and advance care planning discussions
  • Collaborate with caregivers, facility teams, PCPs, and interdisciplinary partners
  • Educate patients and caregivers
  • Maintain accurate clinical documentation
Ideal Candidate

This opportunity is ideal for Nurse Practitioners and Physician Assistants who enjoy clinical independence, critical thinking, and ownership of patient outcomes. You'll care for patients with complex medical, behavioral, functional, and social needs while having the flexibility and autonomy to shape your daily practice.

Required Qualifications
Nurse Practitioners
  • MSN or DNP from an accredited program
  • Active Nurse Practitioner license (or ability to obtain by start date)
  • AANP or ANCC certification in an approved NP specialty
Physician Assistants
  • Graduate of an accredited Physician Assistant program
  • Active Physician Assistant license (or ability to obtain by start date)
  • NCCPA certification
Additional Requirements
  • DEA licensure/prescriptive authority or ability to obtain
  • Ability to obtain a collaborative practice agreement, if applicable
  • Valid driver's license and reliable transportation
  • Ability to meet the physical requirements of a field-based role
Preferred Qualifications
  • 1+ years of clinical experience, preferably in long-term care
  • Experience caring for patients with complex medical, behavioral, social, or functional needs
  • Knowledge of geriatrics, chronic illness, advanced illness, and end-of-life care
  • Proficiency with electronic medical records and clinical documentation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
