Join Optum Home & Community Care, where we're reimagining how healthcare is delivered to seniors. As part of our Senior Community Care (SCC) team, you'll provide personalized, in-person care to patients in skilled nursing facilities, assisted living communities, and senior housing settings. You'll work with some of the highest-risk members, helping improve outcomes, reduce barriers to care, and make a meaningful difference in patients' lives.

What You'll Do
  • Perform comprehensive health assessments, including physical exams, medication reviews, and chronic disease management
  • Support members during short-term rehabilitation stays and transitions of care
  • Partner closely with facility staff, caregivers, and interdisciplinary teams
  • Conduct advanced care planning and goals-of-care discussions
  • Educate patients and families on treatment plans and health management
  • Ensure accurate clinical documentation and coding
What We're Looking For

Required:

  • Certified Nurse Practitioner (AANP or ANCC) or Physician Assistant (NCCPA)
  • Active, unrestricted state license (or ability to obtain before start date)
  • DEA license or ability to obtain
  • Valid driver's license and reliable transportation

Preferred:

  • Experience in geriatrics, senior living, long-term care, home care, or post-acute settings
  • Passion for caring for medically complex and underserved senior populations
  • Strong collaboration and relationship-building skills
Compensation

Compensation generally ranges from $109,500-$164,000, plus bonus opportunity, with final offers based on location, experience, and qualifications.
