Nurse Practitioner/Physician Assistant, House Calls - Yakima, WA
Optum Home & Community Care, part of the Optum family of businesses, is seeking a Nurse Practitioner or Physician Assistant to join its HouseCalls team in Yakima, Washington. This field-based role provides annual in-home health assessments for Medicare Advantage and other health plan members, helping improve outcomes through preventive care, education, and care coordination.

Position Highlights & Primary Responsibilities
  • Conduct comprehensive in-home health assessments for health plan members.
  • Review medical history, reconcile medications, obtain vital signs, and perform physical examinations.
  • Complete evidence-based screenings and point-of-care testing, as appropriate.
  • Identify and document diagnoses to support care management and treatment planning.
  • Communicate assessment findings to primary care providers to help close gaps in care.
  • Recognize urgent or emergent situations and intervene appropriately.
  • Educate patients regarding disease processes, medications, and treatment compliance.
  • Assess social determinants of health and provide referrals to community resources when needed.
Schedule & Work-Life Balance
  • Flexible provider schedules available between 24 and 40 hours per week.
  • Flexible scheduling allows providers to work between 8:00 a.m. and 8:00 p.m. over a seven-day workweek.
  • No on-call responsibilities.
  • No weekends required.
  • No holidays required.
Required Qualifications

For Nurse Practitioners
  • Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
For Physician Assistants
  • Current NCCPA certification and applicable state licensure.
For All Candidates
  • Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without current disciplinary actions or practice limitations.
  • Active and unrestricted driver's license.
  • Reliable transportation to complete home visits.
  • Ability to work in varying home environments and perform assessments requiring sitting, standing, and kneeling.
  • Compliance with applicable state practice requirements for independent practice authorization, where required.
Preferred Qualifications
  • One or more years of clinical experience, preferably in family practice, geriatrics, or home health.
  • Proficiency with electronic medical records and related technology.
  • Ability to transport equipment weighing up to 30 pounds and navigate stairs during home visits.
  • Strong communication skills with geriatric and Medicare populations.
Compensation & Incentives
  • $40,000 Student Loan Repayment or $30,000 Sign-On Bonus
  • Compensation for this specialty generally ranges from $109,500 to $164,000.
  • Total cash compensation includes base pay and bonus opportunities and is determined by several factors, including local labor market conditions, education, experience, productivity, and performance.
