Nurse Practitioner/Physician Assistant, House Calls - Yakima, WA Optum Home & Community Care, part of the Optum family of businesses, is seeking a Nurse Practitioner or Physician Assistant to join its HouseCalls team in Yakima, Washington. This field-based role provides annual in-home health assessments for Medicare Advantage and other health plan members, helping improve outcomes through preventive care, education, and care coordination. Position Highlights & Primary Responsibilities Conduct comprehensive in-home health assessments for health plan members. Review medical history, reconcile medications, obtain vital signs, and perform physical examinations. Complete evidence-based screenings and point-of-care testing, as appropriate. Identify and document diagnoses to support care management and treatment planning. Communicate assessment findings to primary care providers to help close gaps in care. Recognize urgent or emergent situations and intervene appropriately. Educate patients regarding disease processes, medications, and treatment compliance. Assess social determinants of health and provide referrals to community resources when needed. Schedule & Work-Life Balance Flexible provider schedules available between 24 and 40 hours per week. Flexible scheduling allows providers to work between 8:00 a.m. and 8:00 p.m. over a seven-day workweek. No on-call responsibilities. No weekends required. No holidays required. Required Qualifications For Nurse Practitioners Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care. For Physician Assistants Current NCCPA certification and applicable state licensure. For All Candidates Active, unrestricted licensure in the state of practice (or ability to obtain by start date) without current disciplinary actions or practice limitations. Active and unrestricted driver's license. Reliable transportation to complete home visits. Ability to work in varying home environments and perform assessments requiring sitting, standing, and kneeling. Compliance with applicable state practice requirements for independent practice authorization, where required. Preferred Qualifications One or more years of clinical experience, preferably in family practice, geriatrics, or home health. Proficiency with electronic medical records and related technology. Ability to transport equipment weighing up to 30 pounds and navigate stairs during home visits. Strong communication skills with geriatric and Medicare populations. Compensation & Incentives $40,000 Student Loan Repayment or $30,000 Sign-On Bonus Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus opportunities and is determined by several factors, including local labor market conditions, education, experience, productivity, and performance.
