As a Full-Time Registered Nurse, Home Health, you'll provide skilled nursing care to patients in the comfort of their homes while helping them achieve their health goals and maintain independence. You'll work closely with physicians, caregivers, and interdisciplinary care teams to develop personalized plans of care, coordinate services, and deliver high-quality, patient-centered care. Primary Responsibilities:
  • Provide comprehensive nursing assessments and develop individualized plans of care in collaboration with physicians and other healthcare professionals
  • Deliver skilled nursing services in accordance with physician orders, professional standards of practice, and agency policies
  • Complete and document patient assessments, including OASIS documentation and medication reconciliation, accurately and timely
  • Coordinate care across the interdisciplinary team to support continuity of care and positive patient outcomes
  • Educate patients and caregivers on disease management, medications, treatments, and preventive care strategies
  • Supervise and support home health aides and LPN/LVNs in accordance with state and federal regulations
  • Communicate effectively with patients, families, physicians, and team members regarding patient status, care plans, and clinical needs
  • Participate in quality improvement initiatives, survey readiness activities, and patient safety efforts
  • Participate in on-call and weekend rotations as needed to support patient care needs

Required Qualifications:
  • Current and unrestricted RN licensure in the state of practice
  • Current driver's license, vehicle insurance, and access to reliable transportation or public transportation
  • Current CPR Certification or ability to obtain within 90 days of hire
  • Ability to work effectively with patients of diverse backgrounds, experiences, and needs
Preferred Qualifications:
  • 1+ years of home health experience
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently and manage a flexible schedule
  • Excellent communication, documentation, and organizational skill
Role details
  • Position: Home Health Registered Nurse
  • Coverage area: Northeast Central Indiana, including Wabash, Grant, Huntington, and Blackford Counties.

  • Sign-on bonus: $10,000 for eligible full-time external hires
  • Benefits: Available for eligible employees

Pay Range
$64,100 - $141,500 annual total cash target pay
$36.98 - $81.63 per visit point
$30.82 - $68.03 hourly rate

Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
