About the Role Optum Senior Community Care's SNF Patient Connect Program focuses on supporting Medicare members during short-stay and transitional care episodes in skilled nursing facilities. The provider performs a one-time comprehensive health assessment and works closely with the case management team to improve care coordination, reduce hospital readmissions, enhance documentation accuracy, and support better patient outcomes.

Key Responsibilities

  • Conduct comprehensive health assessments for patients in skilled nursing facilities
  • Collaborate with primary care physicians, nursing staff, families, and case management teams
  • Support care coordination and transition planning
  • Complete documentation, coding, and gap closure activities
  • Help improve quality measures, advanced care planning completion, and patient satisfaction

Required Qualifications

  • Active, unrestricted NP or PA license
  • NP certification through AANP or ANCC, or PA certification through NCCPA
  • Active DEA license or ability to obtain
  • Valid driver's license and reliable transportation
  • Ability to travel to SNF facilities within the assigned territory
  • Ability to transport a 30-pound bag and navigate various patient care environments
  • Availability to work at least 10 hours per week
  • Ability to obtain a collaborative practice agreement if required by state law

Preferred Qualifications

  • 1+ years of post-graduate experience in long-term care or skilled nursing settings
  • Knowledge of geriatrics, chronic disease management, advanced illness, and end-of-life discussions
  • Experience with electronic medical records
  • Strong relationship-building and communication skills
  • Ability to adapt to change and work independently

Highlights This is an autonomous, per diem role caring for Medicare patients in skilled nursing and transitional care settings. Compensation for this specialty generally ranges from $xx.xx - $xx.xx per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
