Schedule

  • Additional compensation available for providers who are fluent in a language other than English and use those skills during member visits
  • Flexible schedules available between 24-40 hours per week

About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to provide annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, care gap closure, and collaboration with primary care providers.

Key Responsibilities

  • Conduct comprehensive in-home health assessments
  • Review medical history and medications
  • Perform physical exams, screenings, and point-of-care testing
  • Identify and document diagnoses to support care management and treatment planning
  • Communicate findings to primary care providers
  • Educate members on health conditions, medications, and treatment compliance
  • Address social determinants of health and connect members with community resources
  • Recognize and respond appropriately to urgent or emergent situations

Required Qualifications

  • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • PA: NCCPA certification and active state licensure
  • Active, unrestricted license (or ability to obtain by start date)
  • Valid driver's license and reliable transportation
  • Ability to work independently in a variety of home environments

Preferred Qualifications

  • 1+ years of clinical experience in family practice, geriatrics, primary care, or home health
  • Experience working with Medicare or geriatric populations
  • EMR and technology proficiency
  • Strong communication and patient education skills
  • Ability to transport equipment up to 30 pounds and navigate stairs
  • Fluency in a language other than English

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $109,500 to $164,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
