About the Role

HouseCalls is a unique in-home assessment program focused on delivering preventive health services directly to members. As an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll conduct annual wellness visits in patients' homes, helping identify care gaps, improve health outcomes, and communicate important findings to primary care providers. This is a non-prescribing, field-based position centered on assessment, education, and care coordination.

What You'll Do
  • Perform comprehensive in-home health evaluations, including:
    • Medical history and medication review
    • Physical assessments and vital signs
    • Preventive screenings and point-of-care testing when appropriate
  • Identify and document clinical findings that support treatment planning and care management
  • Partner with primary care providers to help close gaps in care
  • Recognize urgent clinical concerns and take appropriate action
  • Educate members on chronic conditions, medications, and healthy lifestyle practices
  • Address social determinants of health and connect members to community resources when needed
