Position Details:

  • Location: Hybrid position, based out of 3 Farm Glen Boulevard, Farmington, CT, 06032
  • Department: Case Management
  • Schedule: Full time, 40 hours/weekly. Monday through Friday, hours 8:00AM - 4:30PM. Hybrid schedule requiring 2-3 days in office depending on team needs. On rare occasion, may flex to include evening hours to 7:00PM

Primary Responsibilities:

  • Actively manages ProHealth inpatient/outpatient hospital census lists to conduct transition in care outreach
  • Directly engages patients to ensure follow-up appointments are scheduled, discharge instructions understood, and medication reconciliation
  • Communicates as needed with Regional Medical Directors, providers, practice managers and other care team members to identify and help solve for barriers to care or gaps in care
  • Document transition care activities in patient electronic health record
  • Initiates second level review regarding complex patient needs with a RN Transition Care Coordinator
  • Engage patients using motivational interviewing techniques through phone calls or other methods
  • May be asked to participate in cross training in order to function in multiple roles as required
  • Collaborates with Case Managers and other staff, both in the central office and at practice/payer sites, to ensure outreach efforts are aligned and not duplicative
  • Perform other duties as assigned

Required Qualifications:

  • Professional education in nursing
  • Active, current and unrestricted LPN licensure in CT
  • 3+ years in acute, primary or hospital setting
  • Solid Microsoft Office Suite (Excel, and Word) experience
  • Basic computer knowledge and aptitude
  • Reliable transportation

Preferred Qualifications:

  • Experience with EMR systems (EPIC)
  • Experience in motivational interviewing

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
