As a Patient Care Coordinator (PCC), you'll serve as a key member of our primary care team, collaborating closely with nurse practitioners (NPs), physician assistants (PAs), and other interdisciplinary team members. This role supports multiple providers and plays a critical part in ensuring patients receive coordinated, high quality care.
This is a field Based role, offering flexibility to support work life balance while meeting business and patient needs. PCCs work both from home and onsite in facilities. While travel to facilities is required, it is not expected every day, and team members may work from home several days per week when appropriate.
Optum strives to assign work locations as close to home as possible, when feasible, to support work life balance-while still meeting operational needs.

This position is full-time Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime.

We offer 2 months of paid training. The hours of the training will be based on schedule or will be discussed on your first day of employment.

Primary Responsibilities:

  • Support the interdisciplinary care team with overall member coordination under the direction of an RN or NP/PA
  • Perform data entry and manage patient charts, including:
    • New members
    • Post hospital visits
    • Quality initiatives
    • Labs and medications
  • Obtain medical records following hospitalizations or physician appointments
  • Schedule patient transportation and medical appointments
  • Serve as a flexible team member supporting both providers and patients
  • Building relationships within nursing facilities to supplement the programs mission

Work Location, Travel & Mileage Reimbursement

  • This position requires travel to patient care facilities, though daily field visits are not required. You will spend 40% of your time in the field and 60% at home.
  • Candidates must have reliable transportation and be comfortable traveling as needed
  • Mileage reimbursement is provided for the use of a personal vehicle in accordance with company policy
  • Candidates living within Baltimore or Anne Arundel counties, including those near county lines, will be considered, provided they are comfortable with drive times to assigned facilities

Required Qualifications:

  • High School Diploma / GED OR equivalent work experience
  • Must have valid driver's license
  • Must be 18 years of age OR older
  • 3+ years of clinical or medical records experience, including knowledge of medical terminology and EHR platforms
  • Access to reliable transportation
  • Must reside in Baltimore or Anne Arundel counties (including county line residences if comfortable with travel)
  • Ability to work full time, Monday-Friday, between 8:00am 5:00pm.

Preferred Qualifications:

  • Certified Nursing Assistant (CNA) or Medical Assistant (MA) certification

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $20.38 - $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
