Overview: Advanced Practice Nurse - Primary Care

This full-time position will work with the primary healthcare team to ensure individuals with a mental illness or substance use diagnosis are able to obtain primary medical care within the standard of care for Internal Medicine patients. THIS POSITION IS ELIGIBLE FOR A $10,000 SIGN ON BONUS with a two-year employment commitment.

Core Responsibilities Include:

• Perform physical examinations of patients to evaluate health and diagnose illnesses
• Determine, plan and initiate appropriate treatment, and adjust treatment if necessary
• Prescribe and dispense medications
• Documentation of medical information in electronic medical records
• Educate and counsel patients and their families on presenting health problems, treatment plans, and home care
• Encourage and promote preventative health care and positive health behaviors
• Request consultation or referral with other health care providers
• Actively maintain knowledge of current medical trends, including medication use and possible side effects

Education and Experience:

• Graduate degree from an accredited nursing school (Master of Science in Nursing)
• Current licensure in the State of Illinois (in good standing and without prior disciplinary action) is required
• A valid driver's license, reliable means of transportation, and proof of current automobile insurance are required.

Salary Range: $110,000-$135,000 per year (dependent on education and experience)
