Hospice Admission RN

We are searching for a Registered Nurse - Supportive Care to perform a variety of duties within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physician's orders, Plan of Care and Hospice and Home Care policies and procedures. Additional responsibilities could include case management and overflow visits.

• Hours: 40hrs
• Shift: Day/Evening shift with weekend and holiday rotation; 12pm - 8:30pm
• Travel: This is a home care position. Will require driving

Education and/or Experience Requirements:

• Graduate of an NLN accredited school of Nursing is required.
• Bachelor of Science in Nursing is preferred.
• MA and RI I License Required
• Two years recent hospital, medical/surgical or other relevant nursing experience required.
• Hospice & Palliative Care Certification upon eligibility or in 2 years.
• Demonstrated knowledge of I.V. pump, and central line maintenance preferred.
• Has knowledge and skills necessary to provide care, which is appropriate to the population of patients served.
• Proficient computer skills, including experience with various software, database and electronic records programs is preferred.

• $9,600 New Hire Sign On with 1-3years of Hospice/Palliative experience
• $8,000 New Hire Sign On with 1+ years of Hospital RN Experience OR Less than 1 year Hospice/Palliative experience
• Rehires termed greater than 1 year are eligible
