Program Coordinator RN - Home-Based Hospital Care

Work Schedule: .875 FTE, four 10-hour shifts one week, three the next from 0800-1830 with an every other weekend and holiday rotation. Hours are based off the operational needs of the department. This position is hybrid with onsite work and travel required within the Madison, WI area. Your assigned work locations may include: the Administrative Office Building at 7974 UW Health Court, Middleton, WI, the Administrative Services Building at 301 S. Westfield Rd., Madison, WI, and/or 3209 Dryden Drive in Madison, WI.

Pay:
Additional components of compensation include impressive shift differentials - $4/hour evening, $5/hour night and $4.25/hour weekend differential as applicable for hours worked.

Come coordinate care for Home-Based Hospital Care (HBHC) patients, serving as the central point of contact through virtual visits and phone communication to ensure seamless, hospital-level care in the home.

We are seeking a Registered Nurse to:

• Partner closely with the rounding provider, equipment coordinator, admission RN, and in-home RNs/paramedics within a care team model to support daily care planning, patient monitoring, and safe transitions of care.
• Conduct virtual medication administration visits and ongoing virtual patient check-ins, providing clinical oversight and addressing changes in condition in collaboration with the interdisciplinary team.
• Participate in interdisciplinary rounds and manage post-acute referrals, discharge services, and equipment coordination to support safe discharge and continuity of care.
• Maintain thorough and timely documentation in the electronic medical record while monitoring patient progress and ensuring individualized care needs are met throughout the HBHC episode.
• This is a hybrid position combining onsite and remote work; infrequent travel to University Hospital or East Madison Hospital may be required to support patient enrollment.
