The Nurse Triage role involves assessing patient symptoms and concerns over the phone or through electronic communication and determining the appropriate level of care needed. The triage nurse provides clinical advice, directs patients to the appropriate services, and collaborates with healthcare providers to ensure timely and effective patient care within a medical office setting.

Key Responsibilities:

• Telephone Triage:
  • Answer incoming calls from patients with clinical concerns.
  • Assess symptoms using standardized protocols and clinical judgment.
  • Provide safe, evidence-based advice and determine urgency of care.
  • Direct patients to appropriate care level: self-care, office visit, urgent care, or emergency department.
• Clinical Documentation:
  • Accurately document patient interactions, assessments, and recommendations in the electronic medical record (EMR).
  • Ensure clear communication with providers regarding triage outcomes.
• Patient Education:
  • Offer education about symptoms, home care, medications, and follow-up instructions.
  • Reinforce care plans provided by providers.
• Coordination of Care:
  • Schedule urgent or same-day appointments when needed.
  • Facilitate communication between patients and providers.
  • Support medication refills and prior authorization requests when appropriate.
• Collaboration:
  • Work closely with Clinical Manager, Nurses and office staff to ensure coordinated patient care.
  • Participate in clinical team meetings as required.

Job Description

Minimum Qualifications:

1. Massachusetts RN Licensure.

2. Two (2) years of clinical nursing experience.

3. Current Basic Life Support (BLS) Certification.

Preferred Qualifications:

1. Bachelor of Science in Nursing (BSN).

2. Three (3) years of clinical nursing experience.

3. Department Specific: Certification(s) relevant to the department and the type of patient care being provided.
