GYN/OB Social Worker II

Join our GYN/OB ambulatory care team as a Social Worker II, providing compassionate, family-centered psychosocial care to patients navigating acute and chronic women's health, gynecologic, obstetric, and reproductive health conditions. This role plays a vital part in supporting patients and families through comprehensive psychosocial assessment, brief therapeutic intervention, crisis support, and care coordination to help improve health outcomes and treatment adherence.

The Social Worker collaborates closely with an interdisciplinary team to address the social, emotional, behavioral, and environmental factors that impact care. This position requires strong knowledge of healthcare, behavioral health, educational, child welfare, and community service systems, as well as the ability to connect patients and families with appropriate resources and supports across settings. The ideal candidate will bring cross-system experience in both clinical and community-based environments, with the ability to build collaborative relationships that promote continuity of care and strengthen access to culturally responsive services.

Spanish language proficiency is preferred.

Key Responsibilities



  • Conduct comprehensive biopsychosocial assessments for GYN/OB patients and families
  • Provide brief therapeutic interventions, supportive counseling, and crisis assessment
  • Develop and implement safety plans as needed
  • Support patients and families with adjustment to illness, diagnosis, treatment, pregnancy, delivery, postpartum care, and care transitions
  • Offer caregiver support, advocacy, and education
  • Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
  • Identify psychosocial barriers to care and assist with treatment adherence and care planning
  • Connect patients and families to appropriate community resources and support services
  • Maintain collaborative relationships with community partners to support coordinated care
  • Document assessments, interventions, and care plans accurately and timely in the electronic medical record
  • Perform other duties as assigned


Qualifications

Education



  • Master of Social Work (MSW) required


Licensure



  • Valid LCSW-C required


Knowledge, Skills, and Abilities



  • Strong understanding of the psychosocial impact of illness on patients and families
  • Experience working in cross-system environments, including clinical and community-based settings
  • Ability to work independently in a healthcare or mental health setting
  • Strong assessment, counseling, and problem-solving skills
  • Ability to develop comprehensive biopsychosocial assessments and treatment plans
  • Knowledge of community resources and public service systems
  • Excellent communication, documentation, and organizational skills
  • Proficiency with electronic medical records and standard office systems
  • Ability to build effective, collaborative relationships with patients, families, internal teams, and community partners


Work Environment



  • This position does not require regular evening or weekend hours.


Salary Range: Minimum 34.62/hour - Maximum 57.19/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
