Job Description:

Conduct nutritional assessment of patients with diabetes, and develop nutrition plan. Educate patients, families, staff and other caregivers related to nutrition in diabetes management.

Principle Responsibilities
  • Assesses and plans for nutritional needs of patients with diabetes.
    • Conducts nutrition assessment.
    • Develops nutrition plan.
    • Collaborates with physicians as needed to identify nutritional and / or diabetes management needs and plan of care.
    • Utilizes approved protocols for medication management to advance the plan of care when appropriate.
    • Provides consultation and mentoring related to diabetes care to staff in complex cases and new or unfamiliar clinical problems.
    • Partners and collaborates with multi-disciplinary team.
    • Plans and documents discharge needs for patient and family.
  • Conducts patient and family education.
    • Provides nutrition education to patients and their family.
    • Evaluates patient and family response to education.
  • Documents in medical record.
    • Documents assessment, plan, interventions, education and outcomes in medical record.
    • Documents nutritional goals and data in the medical record.
    • Documents to assure accurate coding and billing for services.
    • Maintains documentation tools, educational literature and resources.
  • Other duties as assigned.

Preferred Qualifications
  • Bachelor's degree in Dietetics or related field
  • 2 to 5 years of experience in treatment of diabetes patients in hospital or nursing home setting
  • 2 to 5 years in documentation in electronic medical record

Licenses/Certifications
  • Licensed Dietitian - Minnesota Board of Dietetics and Nutrition Practice required Or Licensed in Wisconsin for WI practice settings upon hire
  • Registered Dietitian - Commission on Dietetic Registration
  • Academy of Nutrition and Dietetics preferred Or American Dietetic Association Registration upon hire
  • Certified Diabetes Educator - National Certification Board for Diabetes Educators required within 4 years
  • American Dietetic Association Registration upon hire preferred
  • Licensed Dietician-Wisconsin for Dieticians in Wisconsin practice setting upon hire required

Pay Range

Pay Range: $38.52 to $52.80 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
