What you can expect Location: 2301 Erwin Rd. Durham, NC 27705 The Pediatric Transplant and Cellular Therapy (PTCT) Nurse Navigator serves as a key clinical liaison and care coordination resource for pediatric and young adult patients after transplant, cellular therapy, or related advanced therapies. This Monday through Friday, daytime clinic-based role supports longitudinal follow-up, proactive care navigation, and patient- and family-centered coordination across the post-transplant continuum. The Nurse Navigator works closely with the multidisciplinary PTCT care team, including physicians, advanced practice providers, nurses, social work, administrative partners, insurance authorization teams, and primary care providers. The role is designed to support timely access to care, clear communication, treatment adherence, care transitions, and ongoing surveillance for medically complex pediatric post-transplant patients. This position reports to the Nurse Manager and receives clinical and operational support from the Pediatric Transplant and Cellular Therapy team. Schedule:
