PEDS TRANSPLANT & CELLULAR THERAPY NURSE NAVIGATOR

The Pediatric Transplant and Cellular Therapy (PTCT) Nurse Navigator serves as a key clinical liaison and care coordination resource for pediatric and young adult patients after transplant, cellular therapy, or related advanced therapies. This Monday through Friday, daytime clinic-based role supports longitudinal follow-up, proactive care navigation, and patient- and family-centered coordination across the post-transplant continuum.

The Nurse Navigator works closely with the multidisciplinary PTCT care team, including physicians, advanced practice providers, nurses, social work, administrative partners, insurance authorization teams, and primary care providers. The role is designed to support timely access to care, clear communication, treatment adherence, care transitions, and ongoing surveillance for medically complex pediatric post-transplant patients.

This position reports to the Nurse Manager and receives clinical and operational support from the Pediatric Transplant and Cellular Therapy team.

Schedule:

• Monday through Friday, daytime hours.

• Provide individualized care navigation and clinical support for pediatric post-transplant patients and families across the outpatient continuum.
• Serve as a consistent point of contact and primary liaison among patients, families, the multidisciplinary PTCT team, primary care providers, insurance authorization partners, and administrative teams.
• Coordinate longitudinal follow-up care, including clinic visits, laboratory monitoring, imaging, referrals, survivorship needs, and transition points in care.
• Conduct nursing assessments, triage patient and caregiver concerns, and facilitate escalation to the appropriate clinical team member or level of care.
• Support patient and family education related to post-transplant expectations, medication adherence, infection prevention, symptom monitoring, follow-up schedules, and available resources.
• Identify and help address barriers to care, including care access, scheduling, insurance authorization, transportation, communication, and psychosocial needs.
• Partner with clinical and operational leaders to improve workflows, communication, patient experience, quality, safety, and continuity of care.
• Maintain accurate documentation and promote timely, clear communication among care team members and external partners.

• Bachelor's degree in Nursing is required; Master's degree in Nursing is preferred.
• Current or compact Registered Nurse licensure in the state of North Carolina is required.
• Minimum of three years of nursing experience is required.
• Pediatric nursing experience is preferred.
• Experience in nurse navigation, care coordination, oncology, hematology, transplant, cellular therapy, or management of medically complex pediatric patients is strongly preferred.
• BCLS certification required.
• Demonstrated ability to communicate effectively.
• Strong clinical assessment, triage, prioritization, organization, and problem-solving skills.
• Ability to support longitudinal care, anticipate patient and family needs, and coordinate across complex systems of care.
