JOB DESCRIPTION: The Multilingual Community Health Coordinator builds trusted relationships with multilingual individuals, families, and communities and helps them navigate health care and social-service systems across UVM Health's service area in Vermont and Northern New York. The position provides culturally and linguistically responsive outreach, education, advocacy, resource connection, and follow-up to help address language, financial, transportation, digital, and other nonclinical barriers to care. The Multilingual Community Health Coordinator also co-chairs the Multilingual Patient and Family Advisory Council, helping ensure multilingual patient and family perspectives inform service improvement, community health planning, and organizational decision making. Serving as a bridge among communities, UVM Health teams, and community-based organizations, the Multilingual Community Health Coordinator helps individuals access appropriate services, provides warm handoffs to health and social resources, and connects patients and families with qualified interpreter services and translated materials. The position communicates directly in languages in which the employee has demonstrated proficiency for routine outreach, education, and navigation but does not provide medical interpretation or independently translate regulated materials unless separately qualified and authorized. The Multilingual Community Health Coordinator also identifies recurring community needs and access barriers, shares community feedback with appropriate teams, and supports Community Health Needs Assessment and Community Health Improvement Plan priorities affecting multilingual communities. The position reports to the Community Health Improvement Manager and works in alignment with UVM Health's broader community health improvement and health equity priorities. EDUCATION: Associate degree or additional education in community health, public health, human services, social services, education, or a related field preferred.
