As The Care Team Coordinator (CTC), you will be responsible for a client care team at Main Line Health Home Care. You will be responsible for the coordination of clinical care across the disciplines and health care providers. You will also work in conjunction with the Operations Manager to develop and monitor client care services based on customer needs and oversee the day to day operation of the care team. Position: Physical Therapist Care Team Coordinator - Home Care Therapy Shift: Full Time, 40 hours/week, Day Shift Experience: 1. Minimum 3 years of physical therapy clinical experience in a home care position 2. Minimum on 2 years of management/leadership experience preferred. Education: 1. Bachelor's degree in allied health field required, master's degree preferred. 2. 3-5 years recent clinical experience in an allied health profession or home care required. 3. Management/Supervisory experience preferred. Licensures/Certifications: 1. Licensed in Commonwealth of PA in clinical profession required, PA Physical Therapist License 2. Provider BLS (American Heart Assoc/American Red Cross) required. Employee Status: Regular Schedule: Full-time Shift: Day Job Pay Range: $93,350.40 - $145,641.60 Job Grade: 113 | |
