Position Summary:

We have an exciting opportunity to join our team as a Nurse Practitioner in Kidney Transplantation/ Xenotransplantation Research in Manhattan.

The Nurse Practitioner, Xenotransplant will manage high-acuity patients, coordinate multidisciplinary care, and develop and prescribe comprehensive treatment plans. Additionally, the Research Nurse Practitioner will be responsible for provision, coordination, and assurance of comprehensive advanced practice medical care using pertinent standards of care, illness diagnosis of physical conditions, and performance of therapeutic, corrective, and prescriptive measures as appropriate for preoperative and postoperative transplant patients, including patients undergoing xenotransplantation. Advanced expertise in clinical reasoning, data-driven decision-making, and evidence-based practice will also be performed.

Job Responsibilities:

Supports the Transplant Institute and the medical center in all transplant research efforts, with a focus on xenotransplantation.
Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise.
Assists in the preparation, implementation, and evaluation of transplant research protocols when applicable.
Performs complete health history, physical examination, and psychosocial assessment of pre- and post-operative transplant patients, primarily xenotransplant patients, in the outpatient clinic.
In collaboration with physicians, orders and coordinates laboratory, radiological and diagnostic studies for routine follow up, and as a response to problems identified in the outpatient clinic.
Reviews testing differentiates between normal/abnormal findings and follows standard practice for abnormal findings, which may include further diagnostic testing or referral to consulting physicians per customary pattern of the collaborating physician.
Maintains contact with assigned patient caseload between visits, based on their particular needs. Initiates appropriate response to issues and problems identified during these contacts.
Formulates a plan of care, along with the attending physician, consulting physicians, the research team, patient, family, and other members of the interdisciplinary team for both preoperative and postoperative transplant patients, based on expected goals of care and length of stay, care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes.
Works closely with the research team, research billing compliance team, and hospital finance team to ensure compliance with research billing.
Works closely with insurance case managers and patients to facilitate approvals for transplantation and needed services and medications, as appropriate.
Coordinates discharge planning with attending physicians, inpatient nurse practitioners, social work, the research team, and patient and family to assure timely and safe discharge. Orders / prescribes discharge medications according to New York State law, including controlled substances with DEA authorization.
Prescribes/orders medical equipment, devices, physical and occupational therapy, and home health services per practice protocol.
For nurse practitioners, practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges.
Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Educates patient and family to facilitate and promote healthy adjustment to the transplant process.
Acts as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care. Considers needs and behaviors of specific patient age and cultural groups in all patient care.
Supports the mission, philosophy, standards, goals and objectives of the institution, department, and service line.
Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members.
Participates in the development and revision of interdisciplinary and nursing standards, patient, and family education materials as relevant to area of practice and expertise. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, house staff, other transplant team members, other disciplines, and ancillary services. Works collaboratively with nursing leadership on the transplant unit and elsewhere, to ensure timely communications with the nursing staff.
Provides consultation about the transplant process, including xenotransplantation, to the nursing staff responsible for the patients care in the hospital setting. Facilitates the continuity of care across the continuum of services, e.g., home care, other facilities, appointments with physicians, etc.
Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities.
Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Functions as a respectful member of the health care team.
Promotes own professional growth and development in clinical and managerial role.
Maintains current expertise in transplantation.
Serves as a clinical expert and resource for the education of peers, staff members on the transplant unit and other health professionals.
Participates in educational and outreach programs for transplant patients and their families, and other health professionals to increase knowledge about transplantation, including xenotransplantation.
Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement.
Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service.

Minimum Qualifications:

To qualify you must have:

• Licensure: Current registration as Professional Nurse in New York
• Education: Bachelors degree in nursing, masters degree in nursing
• Certification required: BCLS, ACLS
• Certification/Screening is optional
• Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities.
• Ability to work under supervision.
• Proficiency in Microsoft Office applications, including Word, Excel, Access, PowerPoint, and Outlook.
• Familiarity with Internet applications.
• Ability to identify, analyze, and solve problems.
• Strong time management skills and the ability to work well under pressure.
  
  Required Licenses: Registered Nurse License-NYS, Nurse Practitioner-NYS, Basic Life Support Cert

Preferred Qualifications:

• Knowledge of transplantation.
• Prior experience working with research protocols.
• Prior experience in health care settings.
• Prior experience working in research lab or related clinical setting.
• Experience in an academic medical center environment.
• Membership in NATCO, ITNS desirable.
