Department: MC: Transplant Core Job Summary: The Coordinator RN Transplant 1 performs the nursing process (assessment, care planning, intervention, evaluation) independently and with high level of expertise within the nursing scope of practice for pediatric and adult patients with various stages of single solid organ failure. Evaluates assigned patients for pre transplant work up and reviews data to ensure the patient meets inclusion criteria. Coordinates the initial intake evaluation meeting with the healthcare team, patient, and family. Provides pre and post transplant education to transplant candidates, regarding transplant evaluation process, waiting period until suitable organ is found, immunosuppressive therapy and side effects, signs and symptoms of organ rejection, importance of commitment and compliance to long term and postoperative follow-up. Seeks assistance from Coordinator RN Transplant 2 and or 3 when necessary. Coordinates pre-transplant hospital admissions and alerts involved departments to ensure transplant teams and OR suites are set up and ready. Participates in developing the interdisciplinary plan of care follows post-transplant patients closely to assess for signs/symptom of possible rejection. Works collaboratively with the patients/families, physicians, case managers and social workers regarding discharge and utilizes multiple discharge and continuity of care resources as indicated. Coordinates post-transplant follow-up care in the clinic environment. Recaps discharge teaching, answer questions Keeps transplant team informed of patient's progress. Skillfully applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in cardiac disease and solid organ transplantation. Performs other duties as needed. Education and Experience: Associate's degree in nursing required. Bachelor of Science Degree in Nursing (BSN) preferred. Minimum two years nursing experience in the field of renal disease, liver disease or transplantation. Knowledge and Skills: Demonstrates advanced competencies and knowledge in kidney, pancreas and liver disease and transplantation. Skillfully applies theoretical concepts to clinical practice and continually expands knowledge base and clinical skills with a focus in kidney, pancreas and liver disease and transplantation. Basic knowledge of healthcare regulations and accreditation required. Exhibits exemplary professional behavior at all times. Able to: speak, read and write legibly in English (and Spanish preferred) with professional quality; use computer, printer, and software programs necessary to the position, e.g., Word, Excel, Outlook, PowerPoint, electronic medical record, electronic event reporting program; troubleshoot and calibrate patient care equipment; perform technical patient care activities; relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction; think critically; work independently with minimal supervision; perform basic math and statistical functions; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch. Licensures and Certifications: Active California Registered Nurse (RN) license required. Basic Life Support (BLS) certification issued by the American Heart Association required.
