Summary: The Oncology Clinical Care Coordinator (OCCC) is responsible for coordinating and managing patient care activities to ensure the delivery of high-quality, patient-centered care. This role involves working closely with patients, families, and the healthcare team to facilitate optimal care and resource utilization. This position will serve as a member of the Oncology Support Services team and will work closely with navigation, social work, and financial navigation. The OCCC will support the front desk/patient access team with clinical matters/questions. This role will report to the Manager of Oncology Clinical Services with a dotted reporting structure to the Manager of Oncology Operations. The OCCC is intended to be patient facing and directly engage in check-out for advanced scheduling (ex: infusions, radiation oncology treatments, injections, imaging, same day multi-service appointments, etc.) This position will be responsible for assisting with incoming and outgoing referrals to include prior authorization needs. In addition, the OCCC is responsible for managing critical clinical work queues and serves as a centralized communication point for providers, clinic nurses, triage nurses and front desk staff to streamline communication for a defined list of clinical items pertaining to the patient care continuum. Responsibilities: *Collaborate with the patient access team, triage nursing team, and oncology support services team to streamline the continuum of care for patients actively under treatment. *Provide an additional layer of support for high-risk patients to help reduce or resolve complex barriers to care. *Serves as the central hub for scheduling advance/complex treatment regimens such as infusions and radiation treatments. *Maintain accurate and timely documentation in the electronic health record (Epic) to ensure continuity of care and compliance with regulatory requirements. *Act as a liaison between patients, families, and the healthcare team. Facilitate communication and collaboration to ensure comprehensive and coordinated care. *Will assist with the coordination of outgoing referrals and subsequent prior authorization as needed. *Serves as the subject matter expert for infusion, injection, and acute care clinic scheduling. *Serves as the subject matter expert for clinical decision making for front end/patient access staff. *Serves as a liaison for physicians and other clinical staff when barriers to care are addressed. Other information: Required *Graduate of an accredited nursing program, with current RN licensure in the state of North Carolina *Basic Life Support (BLS) certification *Two (2) years of clinical nursing experience
Preferred *Graduate of an accredited BSN program, with current RN licensure in the state of North Carolina *Four (4) years of clinical nursing experience; preferably in an oncology setting *Experience with infusion scheduling.
Strong organizational, communication, and interpersonal skills. Proficiency in using electronic health records (Epic preferred).
