Behavioral Health Integration Care Coordinator

Summary:

This position will serve as a coordinator of care for patients and families with behavioral health needs identified in Akron Children Hospital Pediatric Clinics (ACHP). The Care Coordinator will work with both the patients and families as well as the entire treatment team to assess the needs, aid with coordinating care to address these needs and monitor progress. Services will be provided in person and remotely to allow for cross coverage of multiple ACHPS.

Responsibilities:

  1. In conjunction with Primary Care Provider, identify and triage patients who are at acute risk of suicide and arrange for transportation to appropriate crisis services.
  2. Participate in regularly scheduled case conferences to coordinate and co-develop treatment plans between team members including PCPs, psychiatry and other providers.
  3. Use evidenced-based tools to monitor symptoms and maintain a registry that tracks treatment progress which includes assisting with identifying patients who are not making expected improvements and aid with coordinating appropriate care.
  4. Assist with navigation between levels of care within Behavioral Health (primary care BH, Specialty OP, PHP, IOP, PIRC)
  5. Work with family to arrange for behavioral health appointments and identify barriers to engagement in treatment and engage in problem solving to address barriers. Conduct basic motivational interviewing interventions to assist in addressing barriers.
  6. Provide education about behavioral health conditions to the patients and families and support medication treatment as indicated.
  7. Assess for social determinants of health and support referrals. To aid with this, coordinate/collaborate with school health or other community organizations as needed.
  8. Document in medical record
  9. Works effectively with patients and families of diverse backgrounds.
  10. Other duties as assigned.

Other information:

Technical Expertise

  1. Experience working with multidisciplinary teams.
  2. Proficiency in Electronic Health Record computerized documentation is preferred.
  3. Experience in healthcare is preferred.
  4. Excellent communication skills, both verbal and written, coupled with strong listening skills.
  5. Strong organizational skills with the ability to multi-task, as well as anticipate and resolve problems.
  6. Proficiency in MS Office (Outlook, Excel, Word, Power Point) or similar software is required.

  1. Education: High School diploma or equivalent is required.

  3. Certification: N/A
  4. Years of relevant experience: 2-4 years' experience. A bachelor's degree in a related field can be substituted for years of experience.
