We're looking for a Care Team Specialist (CTS) to join our collaborative, patient-centered clinic team. In this role, you'll play a key part in supporting both clinical and administrative workflows to ensure high-quality care and a smooth patient experience. The Care Team Specialist (CTS) Urgent Care participates as a member of a care team and supports high quality patient care including engaging patients in maintaining their physical and mental well-being. The Care Team Specialist Urgent Care performs technical, clinical, and administrative tasks which contribute to the smooth workflow and quality patient care experience in the clinic. Responsibilities include, but are not limited to pre-visit preparation, monitoring patient flow, rooming patients, obtaining patient histories and vital signs, managing certain test results following protocols, supporting Epic in-basket workflow, returning phone calls and assisting with medical examinations and procedures.. The Care Team Specialist Urgent Care assists and collaborates with the Registered Nurses and Medical Providers in the management and coordination of patient care. The CTS UC staff will participate in the review of team/provider in-baskets and My Chart messages for the assigned care team. Practitioner and Registered Nurse staff may delegate assignments to the Care Team Specialist UC. CTS UC staff at GHC-SCW will have successfully completed medical assistant training or licensed practical nurse training or Paramedic training from an accredited program.
