The Patient Care Navigator (PCN) serves as a primary liaison between healthcare providers and individuals recovering from limb loss. The PCN delivers superior patient care and customer service via proactive intervention, communication, and providing access to emotional support and educational resources.

The PCN should be equipped with a thorough understanding of prosthetics, orthotics, and the medical/healthcare field. The PCN offers patient consultation and guidance from the initial patient referral to prosthetic delivery, serving as a single point of contact as they transition through the continuum of care.

Working closely with Hanger prosthetic personnel, office staff, and Business Development Mangers this position holds responsibility for the day-to-day management of, and communication with, patients, internal team, and health care providers involved in the care of the patients they are managing. Through interacting with healthcare providers in the community, the PCN will be responsible for maintaining current referral relationships, building loyal referral partners and fostering new opportunities to serve more patients



Your Impact

    Manage new amputees along the amputee care pathway from pre/post-surgery to ambulation by offering resources, peer support and guidance throughout their recovery process.
  • Serve as a liaison between patients and their families, referring physicians and other healthcare professionals by providing information regarding the clinic's services to ensure continuity of care and patient/family needs are met.
  • Ensure that new patients are contacted before or after initial consultation or AmpuShield fitting and are entered into Hanger Clinic's patient management and tracking system.
  • Utilize tracking tools to manage patients and document progress.
  • Coordinate activities involved in pre/post-operative planning, preparation and care coordination throughout the delivery of a definitive device.
  • Keep patients engaged after initial contact with follow-up communications and appointments to ensure excellent patient experience and continuity of care at Hanger Clinic.
  • Attend hospital visits, visits to skilled nursing facilities, or rehab facilities as needed and determined by the care team.
  • Apply clinical communication and management skills to collaborate with the healthcare team on follow up visits' w/surgeon including wound description, incision photos, and patient status to provide seamless care delivery to our patients and their families.
  • Work in hospital, skilled nursing and rehab type facilities, patient care settings and complete all hospital/facility vendor credentialing.
  • Help maintain current referral relationships for Hanger Clinic.

Assist with obtaining appropriate clinical documentation for insurance coverage



Minimum Qualifications

  • Required
  • BA/BS in health-related field or equivalent work experience.
  • Ability to be credentialed in major hospital systems.
  • Experience in providing a high level of customer service in a fast-paced environment, including handling confidential data with discretion.
  • Preferred
  • 2 years of experience in a patient care environment
  • Orthotic Fitter/Prosthetic Assistant certification
  • ATC, EMT, MA, Surgical Tech, LVN, PTA, OTA, Social Work or like certification with equivalent experience.
  • Knowledge of prosthetics, orthotics, or durable medical equipment.
  • Experience in case management, medical device coordination, or rehabilitation services.
  • BA/BS in Healthcare Administration, Marketing, Business Administration or another related field OR an equivalent combination of education and experience required
  • Must have, or be eligible to obtain, a valid driver's license and driving record within the standards outlined within Hanger's Motor Vehicle Safety Policy and Procedures.

(Colorado State Only) Pay range of $19.00- $21.00/hour + bonus + benefits. This pay range is posted to comply with Colorado State Law. Hanger salary ranges vary based on skill, ability, knowledge, geographic location and other variables.
