COMMUNITY RESOURCE NAVIGATOR

Chicago Department of Public Health

Number of Positions: 1

(additional vacancies possible pending budget approval)

Starting Salary: $57,000.00/Year

This position is open to the public and all current City employees covered under the City's collective bargaining agreement with AMERICAN FEDERATION OF STATE, COUNTY, AND MUNICIPAL EMPLOYEES (AFSCME) - BARGAINING UNIT 1, 3, 4 & 5. Only employees in City job titles in this bargaining unit are eligible to bid.

Under supervision, provides information and assists clients in navigating and connecting with healthcare and social services

ESSENTIAL DUTIES

  • Meeting with residents or clients (in person or follow up via phone call)

  • Identify the needs of individuals or families and determine appropriate services

  • Connect individuals to appropriate programs and services

  • Increase awareness of available services and build trust in the community

  • Track service delivery and program outcomes such as PlayStreets

  • Maintain strong relationships with service providers

  • Maintain accurate knowledge of community resources.

Performs other related duties as assigned

Location: 111 West Washington Street, Chicago, IL

Days Monday - Friday (May include occasional Saturdays)

Hours 8:30am - 4:30pm

THIS POSITION IS IN THE CAREER SERVICE

Qualifications

Two (2) years of community or social service experience, OR * An Associate's degree or higher from an accredited college or university, OR * Certification as a Community Healthcare Worker or Certified Recovery Support Specialist

Selection Criteria

This position requires applicants to complete an interview. The interviewed candidate(s) possessing the qualifications best suited to fulfill the responsibilities of the position, based on the interview, will be selected.

Preference will be given to candidates possessing the following:

  • Experience in community outreach, public health, healthcare, case management, or social services.

  • Experience connecting individuals and families to healthcare, housing, public benefits, or other community resources.

  • Experience conducting outreach presentations, home visits, or community-based engagement activities.

  • Experience maintaining client records and utilizing electronic databases or case management systems.

  • Bilingual proficiency in English and Spanish or another language commonly spoken in Chicago communities
