Shea Blvd - Leukemia patient care experience highly preferred - Strong understanding of leukemiacare models - BSN preferred but not required - Prior Care Navigation preferredbut not required HonorHealth is looking for a RN Navigator to support the BMT service line focusing on patients needing leukemia care. This role will work with APPs and Providers creating plans of care, education the patient and family members as well as identifying concerns and recommending changes to care. The ideal candidate must meet the minimum requirements of the role of a Associate n Nursing, one (1) year experience in an inpatient/outpatient clinical setting, in addition to at least two (2) years nursing experience. A currentBLS card and current Arizona State RN license and/or Compact license are required. To ensure success in the role prior care navigation, leukemiapatient care experience, and a strong understanding of leukemia care models are valued added to providing the best patient care.Great care starts with great people. Responsibilities: Job SummaryProvides a seamless approach to case coordination by performing multiple responsibilities to ensure optimal patient care. Analyzes the medical record and collaborates with care physicians, patients, family members, and other healthcare team members including case management, to develop a realistic treatment plan. Provides guidance and appropriate resources to patients and families through the assessment of social, financial, and emotional needs. Works collaboratively with appropriate staff to coordinate consultations, referrals, initiation and continuation of home care services, and DME orders. Acts as a patient advocate through the continuum and is available to the physician, patient, and family as a resource to facilitate communication. Essential Functions
