Summary:

The Care Guide coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a role with the interdisciplinary teams to achieve optimal patient-centered, clinical and resource outcomes. Works within care teams to address barriers to treatment, health, wellness, prevention, improvement and outcomes. Serves as an important link between the patient, the healthcare teams, the payers and the community.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Associates Degree in Human Services or 2-3 years of experience in a healthcare setting (or equivalent). Bachelor's level degree in Human Services preferred.
  2. License/Certifications: N/A
  3. Experience: Care Coordination and/or Customer Service experience preferred.
  4. Additional Skills/Requirements Required: N/A
  5. Additional Skills/Requirements Preferred: Effective communication, critical thinking, problem-solving, and customer service skills. Experience in Epic.
