Schedule: Monday - Friday | 8:00 AM - 5:00 PM
Salary: $72,000 annually + Full Benefits Package

Position Overview
Activa Home Health is seeking a highly organized, clinically skilled Registered Nurse (RN) Scheduling Case Manager to support and coordinate care within our innovative Hospital at Home Program. This role blends clinical case management, patient scheduling logistics, and interdisciplinary coordination to ensure seamless delivery of hospital-level care in the home setting.

You will serve as a key liaison between field clinicians, Scheduling Coordinators, and our Virtual Nurse/Command Center in partnership with Cleveland Clinic-ensuring real-time alignment on patient needs, care plans, and service delivery. This role is critical to optimizing clinician utilization, maintaining workflow efficiency, and ensuring timely, high-quality patient care.

The ideal candidate thrives in a fast-paced environment and brings strong experience in home health, intake, scheduling, and care coordination.

Key Responsibilities
* Partner closely with Scheduling Coordinators to manage daily clinician schedules, routing, and coverage needs
* Serve as a clinical liaison between hospital partners, Virtual Nurse/Command Center (Cleveland Clinic), and internal care teams
* Coordinate patient admissions, care transitions, and real-time service adjustments
* Maintain accurate, real-time documentation and workflows within EMR systems (AlayaCare and Kinnser/WellSky preferred)
* Support clinical oversight, staff guidance, and workflow efficiency across the care team
* Participate in quality assurance and performance improvement initiatives

Ideal candidates would have experience with:
* Triage (call center or clinical setting)
* Home Health Intake/Admissions
* Case Management or Care Coordination
* Discharge Planning / Transitional Care
* Clinical Scheduling or Staffing

What You'll Bring
* Active and unrestricted Registered Nurse (RN) license in the State of Florida (required)
* Minimum 3+ years of clinical nursing experience in care coordination strongly preferred.
* Strong understanding of patient intake, scheduling workflows, and care coordination
* Proficiency in EMR systems (AlayaCare and Kinnser/WellSky preferred) and ability to manage real-time documentation
* Excellent critical thinking, communication, and organizational skills
* Ability to manage multiple patients, clinicians, and priorities simultaneously in a fast-paced environment
* Valid Florida driver's license and reliable transportation

Preferred Qualifications

* Experience in acute care, case management, hospital discharge planning, clinical coordinator/scheduler or transitional care management
* Background in multi-disciplinary coordination and utilization management
* Familiarity with Smartsheets or scheduling optimization tools
