Overview
The Mobile Health Services nurse will utilize a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing. Provides healthcare to individuals, families and communities within the RN scope of practice as defined by the Illinois Board of Nursing. This unique nursing opportunity will facilitate care delivery from the mobile health units when deployed out in the community. The mobile health services nurse will work in an environment that requires flexibility, understanding of the dynamics of the community as well as an awareness of cultural dynamics and the delivery of education within these fluid circumstances. This position will work with Leadership to grow and expand Mobile Services and help with oversight of daily operations. In addition, the mobile health nurse will have a knowledge of chronic disease management and be prepared to provide linkages to community and social service organizations for patients meeting the criteria these services. Qualifications Certifications: Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR); Commercial Driver's License (CDL) within 1yr - Illinois Secretary of State (ILSOS)Secretary of State (SOS)Secretary of State (SOS); Basic Life Support (BLS) within 30 days - American Heart Association (AHA)American Heart Association (AHA)American Heart Association (AHA), Education: Nursing Diploma, Work Experience: RN>2yrs Responsibilities Assist with coordination of care for patients of all agesPerform telephone triage of both acute and chronic needs for patients of all agesDevelop linkages with chronic disease specialty services at Carle for referral to the mobile health clinic patientWork closely with entire mobile health clinic team to ensure continuity of care and appropriate follow up for mobile health clinic patientsAdminister immunizations and medications to patients of all agesAssist with oversight of ICARE - VFC/CHIPAssist with Quality oversight of patient care deliveryProvides and facilitates competent health care of patients and families in areas of practice. Ensures adherence to established standards of care and scope of practice. Ensures documentation of care is complete and accurate. Provides health and wellness education to individuals and groups. Acts as preceptor and supports the development of other staff and healthcare students. Provides leadership for initiatives designed to improve the quality of care or clinical outcomes. Assists with the coordination and development of mobile health clinic protocols and policies. Compensation and Benefits The compensation range for this position is $30.39per hour - $52.27per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
