The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: The Infusion Hospital Liaison RN coordinates the nursing component for home infusion therapies from the hospital setting, and acts as a resource person. Participates in program development, quality assurance and the delivery of care. Remains familiar with all pumps, access devices, drugs and central lines. Develops and maintains relationships with referral services and client base. Determines appropriateness of patients for home therapy. Does patient care when needed and on-call. Qualifications: Required:
- Active Registered Nurse (RN) license in the state of New Jersey
- Graduate of an accredited nursing program (ADN or BSN required)
- Current BLS certification
- Valid Driver's license and insured car for travel
- Minimum 2-3 years of RN experience, preferably in infusion, oncology, ambulatory care, or critical care
- IV certification required
- Demonstrated experience with IV therapy and venous access
- Strong clinical assessment, critical thinking, and communication skills
- Ability to work independently while maintaining a patient-centered approach
Preferred:
- Previous Home Care experience
Physical Requirements:
- Ability to stand for extended periods and perform patient lifting/positioning as needed
- Ability to work independently
- Manual dexterity for IV insertion and medication administration
- Ability to respond quickly in emergency situations
Scheduling Requirements:
- Full-Time, Salaried position- 37.5 hours a week
- On-site at RWJ University Hospital New Brunswick
- On-Call participation required 2-3 days per month, with holiday coverage in addition.
- On-Call shift is 5 PM to 8:30 AM via phone triage. Emergency home visits and new admissions as needed.
Essential Functions:
- Assists sales associate when clinical education is needed
- Assesses patient/caregiver for willingness and ability to participate/manage home infusion therapy
- Education and teaching of expectations of participation for ordered therapy done at bedside
- Manages referrals and coordinates patient discharges working closely with case manager, physician, other nursing agencies, RWJBH ISP intake, pharmacists and nurse coordinators.
- Manages patient assignment via scheduling software, prep and calls patient the night before to schedule visit. Informs patient if running late or need for reschedule visits
- Follows physician orders and plan of treatment
- Explain home infusion benefits/coverage and obtain consents for treatment and appeals
- Provides direct patient care in patient's home with ongoing assessment of patient /caregiver willingness and ability to manage/participate in IV home infusion therapy
- Communicate any safety concerns to management team
- Must be comfortable providing care and instructions to both adult and pediatric patients
- Understands and applies age specific communication tools, ensuring patient /caregiver fully understand expectations
- Educates patient and/or caregiver on administration, including pump operation/trouble shooting and signs and symptoms of complications of ordered infusion therapy
- Educate patient and/or caregiver about when/how to call for questions, concerns or complications
- Administer IV infusions, injections, chemotherapy, biologics, parenteral therapies and first doses of antibiotics per physician orders and established protocols
- Educates/ follows safety measures including handling sharps, IV care/precautions and hazardous waste disposal and spillage
- Perform comprehensive patient assessments before, during, and after infusion treatments
- Establish and maintain IV access (peripheral IVs, PICC lines, ports, central lines) as well as manage catheter occlusion when necessary
- Preforms skilled nursing home visits for each assigned patient as clinically indicated which may include blood draw and drop off to lab
- Preforms initial hook up, obtain consents and start education of continuous home infusion therapies in physician office, outpatient clinic and hospital settings
- Monitor patients for adverse reactions and responds appropriately to emergencies
- Educate patients and families on treatment plans, medications, side effects, and post-treatment care
- Reports adverse findings/symptoms to physician and pharmacy team as necessary
- Collaborate with physicians, pharmacists, and interdisciplinary team members to ensure coordinated care
- Maintain infection control standards, medication safety, and infusion best practices including maintaining sterility
- Understands and uses personnel protective supplies as indicated
- Ensure proper handling, storage, and administration of medications and supplies
- Accurately document patient care, assessments, and interventions in the electronic medical record (EMR)
- Participate in quality improvement initiatives and maintain compliance with regulatory standards (Joint Commission, CMS, etc.)
- Remains familiar with the operation, maintenance and troubleshooting of all pumps, supplies and access devices
- Manages/submits payroll and expense reports in a timely manner
- Maintains vehicle safety and maintenance per policy
- Maintains proper supply/nursing bag technique
- Maintains, protects and utilizes company issued computer, phone, iPad etc. per protocol
- Maintains and cleans medical equipment (BP cuff, stethoscope, thermometer etc.) per protocol
- Participates in biweekly nursing huddle.
- Performs other duties as assigned.
