Inova Fairfax Hospital is looking for a dedicated Transplant Coordinator Lead to join the Post-Lung Transplant team. This role will be full-time, Monday - Friday, regular office hours. Sign-on bonus and relocation assistance eligible.

Transplant Coordinator Lead - Thoracic Job Responsibilities:

  • Completes documentation of the plan/orders/outcome related to the pathway according to policy.
  • Delegates patient care activities appropriately. Responsible for aspects of care/communication related to assigned caseload of patients.
  • Educates patient, family, and care providers. Plans, implements, and evaluates formal and informal education regarding all aspects of care for the patient, family and caregivers and documents appropriately.
  • Actively participates in program quality assessment and performance improvement initiatives including committees or teams.
  • Ensures improvements in practice by assuming responsibility for continuing education including obtaining and maintaining professional certification.
  • Coordination of the multidisciplinary care team to implement the plan of care. Keeps team members informed of patient progress/changes.
  • Coordinates orientation for new clinical team members. Identifies education needs and works with clinical manager to support development plans. Serves as program lead for clinical coordinator and clinical support team members. Assists with peer review process.
  • Assists with coordinating team schedules to ensure adequate coverage for daily program needs and on call.
  • Functions as the team's Magnet Champion.
  • Leads program quality assessment and performance improvement initiatives including committees or teams. Functions as a program resource to transplant quality team and program leadership. Monitors team processes for compliance with regulatory requirements including UNOS and CMS standards.
  • Performs other duties as assigned.

Minimum Qualifications:

  • Education: Bachelors Degree in Nursing
  • Experience: 5 years as a Registered Nurse; 2 years in transplant as a RN
  • Certification: Basic Life Support from the American Heart Association; Certified Clinical Transplant Coordinator within 12 months; Licensed in the Commonwealth of Virginia as a Registered Nurse

Preferred Qualifications:

  • Experience: internal team member; experience working in thoracic transplant
