Anticipated Hiring Range: $25.25 - $32.50 hourly
Full Pay Range: $25.25 - $39.75 hourly

Salary offers are based on the candidate's equivalent experience and internal equity with other employees within the same job classification. This position is eligible for overtime compensation for all hours worked over 40 in the designated workweek.

The Social Worker provides case management and care coordination to support individuals at risk for or living with HIV. This position helps clients navigate the healthcare system, access medical care, and connect with essential social services such as housing and food support. In addition, the role delivers education on HIV, hepatitis C, STI's, and prevention strategies, including PrEP and PEP.

The Social Worker also participates in community outreach to engage high-risk and underserved populations, facilitates referrals and linkage to care, provides crisis intervention, and ensures accurate documentation and coordinated care across clinical and social service teams.

We Require

  • Two years of experience as a Social Worker, Patient Navigator, Case Manager, or Health Educator in a health care environment

  • Bachelor's degree in social work or related field

  • Related post-secondary education may substitute for the required experience requirement on a year-for-year basis

Other Requirements

  • Must secure and maintain an Arizona Department of Public Safety Level 1 Fingerprint Clearance Card and complete a background check

  • Within one month of hire, all employees will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test

  • Must possess or have the ability to obtain a valid Arizona driver's license by the time of hire

Job Contributions

  • Conduct interviews, assessments, and observations to obtain relevant background information and determine client needs and eligibility for services

  • Facilitate referral and linkage to care by assisting clients with coordinating medical care, including identifying providers, scheduling appointments, assisting with insurance enrollment, and facilitating transportation

  • Manages an assigned caseload, develops individualized case management and service plans, and conducts longitudinal follow-up to support clients' health and social service needs

  • Provides education and counseling on HIV, Hepatitis C, sexually transmitted infections (STIs), and prevention strategies, including Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP)

  • Identifies individuals at risk for HIV infection and facilitates referrals and linkage to PrEP providers and other prevention services

  • Refers clients diagnosed with or at risk for HIV to appropriate medical providers for diagnosis, treatment, and ongoing care

  • Develops and maintains a current directory of community resources and connects clients with social services addressing needs such as housing, transportation, and financial assistance

  • Participate in outreach and engagement activities targeting high-risk and underserved populations, promoting prevention education

  • Maintain accurate case records and documentation, and serve as a liaison between clients, healthcare providers, and community agencies to support coordinated care

  • Provides emergency assistance and crisis intervention

  • Supports social worker activities across clinical services, including behavioral health assessment, care coordination, and connecting clients to housing, food, employment, and other supportive resources

Working Conditions

  • Ability to lift up to 20 lbs. occasionally, 10 lbs. frequently, or negligible amounts constantly

  • Position is typically office or administrative work and is not substantially exposed to adverse environmental conditions
