Description Department and Program Summary The MaineHealth Preble Street Learning Collaborative (MH PSLC) is a low-barrier clinic that addresses the needs and care of people experiencing homelessness through education, outreach, and walk-in medical services, case management, and other community resources. In 2024, the MH PSLC launched the CONNECT Outreach Van, a grant-funded mobile outreach program that delivers trauma-informed treatment for psychiatric and substance use disorders to people with unstable housing in Greater Portland. The Program Coordinator position will support growth and expansion of the CONNECT program and offers the opportunity to join a close and supportive interprofessional team that provides medical care, social work, and basic support for patients with unstable housing. Helpful Professional Experience Prior work experience in the following areas/topics would be beneficial for this position: * Training in and/or experience with Trauma-Informed Care * Experience working in low-barrier settings * Experience working with people experiencing homelessness * Background in project coordination and management * Background in social work or mental health * Knowledge of substance use disorders Scope of Work The following duties reflect the position's essential functions but do not restrict the assignment of additional tasks. Clinical Administrative Management Manage administrative needs for a patient panel of approximately 50-100 active patients at any given time. Responsibilities include managing authorization forms and medication consent-to-treat documentation; leading daily team huddles to identify patient needs and generate a daily huddle checklist; managing the patient panel calendar to determine when certain medications or tests are due; and conducting periodic reviews of EMR documentation as needed. Referral Management and Communication Develop, implement, and maintain systems for both internal and external referrals to the CONNECT program, ensuring clear communication and appropriate tracking of referral activity. Budget and Expenses Management Workflow Development and Implementation Lead and support the development of clinical and operational workflows and oversee their implementation across the interdisciplinary team to ensure consistency and efficiency. Program Supplies Management Manage procurement, inventory tracking, and re-stocking of essential patient supplies. This includes coordinating supply needs for both the CONNECT Outreach Van and the PSLC clinical department. This also includes supporting the Medical Outreach Case Management team with patient support needs such as pickup of pharmacy prescriptions and Maine Needs orders. Outreach Van Logistics As needed, coordinate daily pickup and drop-off of the CONNECT Outreach Van at the designated MaineHealth parking location and complete required driver training. Manage daily cleaning and re-stocking of Van. Communication and Dissemination Support the Project Director with community partner communications, facilitation of Steering Committee activities, reporting requirements, and dissemination efforts. Additional Program Support Perform other duties as assigned by the Program Director or Clinical Director, or as required by changes in grant or program needs.
