Registered Nurse Case Manager

We are seeking a dedicated and skilled Registered Nurse (RN) to join our team as a Registered Nurse Case Manager. As a RN Case Manager (also known as a Permanent Case Reviewer) [PCR], you will provide the services of Medical Case Management in support of Individual Medical Readiness to the Army Reserve Medical Management Center (AR-MMC). This role shall provide a wide range of services to Soldiers found to have medical issues or concerns that deem them medically not ready (MNR) for mobilization/training and require referral management for care/treatment or for applicable medical board.

PCRs provide guidance and assistance to Soldiers who require referrals for medical treatment or care and assist Soldiers with Individual Medical Readiness (IMR) support to include identification and tracking of Soldiers who are MNR. RN Case Managers collect and review performance data and work with the Army Reserve (AR) Surgeon's office to tailor processes to meet specific USAR wounded, ill and injured Warriors, and coordinate with USAR Recovery Care Coordinators for non-medical case management and family needs, to deliver the highest possible quality of care support.

The PCR will work in conjunction with other healthcare providers, professionals, and non-contract personnel.

Responsibilities:

The PCR will:



  • Work in collaboration with the AR-MMC Intake Team to facilitate identification of the MNR population, ensure assessment of the MNR population and will coordinate with unit leaders and available medical options to determine what the MNR Soldiers need.
  • Assist Soldiers in process to become medically ready or initiate medical boarding packet.
  • Provide guidance and assistance to Soldiers who require referrals for medical treatment or care and assist soldiers with individual medical readiness (IMR) support to include identification and tracking of soldiers who are MNR.
  • Perform intake interview with Soldier and establish communication with Command.
  • Ensure Soldiers understand his/her responsibilities for making and keeping appointments and provide the PCR with medical documentation.
  • Educate the Soldier about their privacy rights and how their information may be used and obtains consent for release of medical information.
  • Manage MNR Soldier's plan of care from receipt of case referral or notification through case closure, as required. In the event of case transition between Contractor staff, PCR will coordinate warm hand off to facilitate continuity of care.
  • Be familiar with and understand regulations pertinent to managing Soldiers who have medical issues and medical readiness without deviation from standard.
  • Compile medical information (profile, periodic health assessment, pertinent medical documentation related to the condition that does not meet retention standard) to compile the Medical Evaluation Board (MEB) packet.
  • Provide entry into the tracking system.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Continually seek improvement to increase client satisfaction, manage day to day operations, and develop lasting relationships with client personnel.
  • Assist in the review process of temporary or permanent P3/4 profiles, contact Soldiers to obtain medical documentation, communicate with Commands and unit as required.
  • Participate in team meetings, trainings, and quality improvement initiatives to enhance decision-making processes and ensure adherence to standards.


Minimum Requirements:



  • Degree: Associates Degree of Nursing.
  • Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE).
  • Experience: Current experience required is three years within the last four years as a registered nurse.
  • Licensure: Current, full, active, and unrestricted license to practice as a registered nurse for the duration of the contract.
  • Foreign Medical Graduates: Contract workers who are graduates of foreign medical schools will possess both a U.S. medical license and certification by ECFMG for health care physicians and Commission on Graduates of Foreign Nursing Schools (CGFNS for nurses as defined in 29 CFR 825.125.
  • All contract healthcare providers shall be required to meet the Continuing Education (CE) requirements as required by their license.
  • Current credentials must be maintained at the assigned worksite and may require periodic updates.


Knowledge, Skills, and Abilities:

Computer skills to enable them to develop working knowledge of Medical Operational Data System (MODS), Medical Protection System (MEDPROS), Medical Readiness Portal, Medical Evaluation Board (MEB) Prep, Joint Legacy Viewer (JLV), Electronic Medical Management Processing System (eMMPS), Integrated Personnel and Pay System (IPPS-A), Personnel Electronic Records Management System (iPERMS), Commander's Strength Management Module (CSMM), Medical Electronic Data (for) Care History And Readiness Tracking (MEDCHART), and Reserve Health Readiness Program (RHRP) as required for performance of services. You will receive training to use these systems.



  • Must be proficient in Microsoft Word, Excel, PowerPoint, Outlook, and Adobe PDF.
  • Ability to communicate effectively orally and in writing.
  • Additional requirements include:
  • U.S. Citizenship.
  • Possess communication skills, problem solving skills, solid background in advanced practice nursing.
  • Strong organizational skills and ability to work independently are also required.
  • Ability to work collaboratively with the Case Management Team and designated military personnel.


WORK SCHEDULE & LOCATION

Work hours are typically 0800 - 1700, Monday through Friday, excluding U.S. Federal Holidays.

The Government may permit telecommuting (teleworking) when determined to be in the best interest of the Government in meeting work requirements.
