The Thompson Center is seeking a dynamic and compassionate Social Worker to join our newly developed Intensive Outpatient Program (IOP) team. This is an exciting opportunity to play a pivotal role in supporting patients and families through coordinated, person-centered care.

Position Overview
As a Social Worker, you will serve as a key collaborator within the IOP service line, coordinating care, optimizing resource utilization, and ensuring patients and caregivers receive comprehensive support throughout treatment. You will work closely with clinical professionals and community partners to promote positive patient outcomes and seamless transitions of care.

Key Responsibilities
* Collaborate with an interdisciplinary team-including behavioral analysts, physicians, psychologists, and therapists-to support individualized care plans based on patient and caregiver needs and preferences.
* Participate in weekly team rounding and maintain consultation availability for in-clinic needs.
* Meet with caregivers within the first two weeks of patient admission to establish a communication plan and conduct a baseline assessment of Social Determinants of Health (SDOH) needs and goals.
* Conduct ongoing needs assessments throughout the patient's participation in IOP to monitor experiences and progress.
* Identify gaps in services, family expectations, and external resources; assist the care team and families in securing appropriate support.
* Support discharge planning by coordinating referrals to post-discharge providers and community resources in a timely manner.
* Assist with planning and procurement of necessary resources or equipment to prevent discharge delays.
* Build and maintain relationships with local community stakeholders to enhance resource networks.
* Complete case management documentation in the electronic medical record (EMR) within established timeframes.
* Maintain working knowledge of applicable regulations, accreditation standards, organizational policies, and department procedures.
* Uphold HIPAA standards and ensure confidentiality of protected health information.
* Report critical incidents and any concerns related to abuse or neglect in accordance with policy and regulatory requirements.
* Perform additional clinical and administrative duties assigned.

Shift

Monday - Friday
8:00 a.m. - 5:00 p.m.

Minimum Qualifications

A Bachelor's degree in related/ relevant area and at least 2 years of experience from which comparable knowledge and skills can be acquired is necessary. Licensure as required for position.

Preferred Qualifications

Three (3) years of clinical experience, which provides knowledge, and understanding of medical terminology, patient diagnoses, treatment protocols and disease process.

Anticipated Hiring Range

Salary Range: $52,915.20 - $83,408.00 annually
Grade: GGS - 9E
