The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Assistant Director of Case Management provides critical support to the Director of Case Management in the overall management and operations of the Case Management Department.

This position plays a key role in ensuring the efficient and effective delivery of patient care through clinical, operational, and leadership support.

Qualifications:

Required:

  • If an RN, BSN required, and Master's degree preferred
  • If a Licensed Social Worker, Master's degree in Social Work required, and LCSW preferred
  • Minimum 4 years of clinical experience in a hospital, home health, or community setting
  • Minimum 2 years experience in acute care case management

Preferred:

  • Previous supervisory experience
  • Certification in Case Management (ACM or CCM) (Certification required within 1 year of hire)

Certifications and Licenses Required:

  • Current and valid license to practice as a Registered Nurse (RN) OR Licensed Social Worker (LSW or LCSW) in the state of New Jersey

Essential Functions:

  • Leadership and Management Responsibilities: Oversees day-to-day operations including management of staff schedules, assignments, and workflow to ensure optimal patient care delivery and resource utilization; Coaches, mentors, and provides regular feedback to staff while ensuring fair and consistent treatment by evaluating performance addressing performance issues, identifying training needs, and supporting professional development; Promotes a positive work environment and foster a collaborative and supportive team environment that values teamwork, communication, and accountability; Participates in hiring case management staff including the recruitment, selection, and onboarding process
  • Clinical Excellence and Patient Care Responsibilities: Oversees the delivery of high-quality, patient-centered care while ensuring adherence to clinical protocols, best practices, and regulatory requirements; Facilitates efficient patient flow by proactively identifying and resolving delays in patient throughput, minimizing length of stay, and optimizing resource utilization; Collaborates with physicians and interdisciplinary team in utilization review escalations; Identifies and mitigates potential risks to patient safety and quality of care
  • Operational Excellence and Compliance Responsibilities: Analyzes data to drive performance improvement; Reviews key performance indicators (KPIs) such as length of stay (LOS), cost per case, readmission rates, and resource utilization to identify areas for improvement; Assists in the development, review, and implementation of departmental and system policies and procedures to ensure compliance with regulatory standards and best practices; Ensures compliance through monitoring adherence to all relevant policies, procedures, and regulations, including those related to patient safety, quality of care, and regulatory compliance; Optimizes resource utilization, including staff, equipment, and supplies, to ensure efficient and cost-effective patient care
  • Communication and Collaboration Responsibilities: Ensures effective communication and coordination among all stakeholders, including physicians, nurses, social workers, therapists, and other members of the care team; Builds strong relationships with internal and external stakeholders (i,e, physicians, payers, and community agencies); Represents the department on interdisciplinary committees and in relevant meetings
  • Professional Development Responsibilities: Sustains professional competence through continuing education and professional development activities; Demonstrates a commitment to professional excellence by serving as a role model for staff; Embraces continuous improvement in performance and professional development; Actively seeks and incorporating feedback from supervisors, colleagues, and staff
