In this role you will, support our patient transitions from inpatient care by coordinating recommended aftercare services based on clinical assessments provided by primary therapists.Key Responsibilities: Coordinate aftercare services based on clinician-recommended levels of care (LOC); does not determine clinical LOC. Collaborate with referral sources to ensure timely, appropriate aftercare placements. Arrange housing and residential referrals, including HWH and RH as indicated. Communicate and coordinate with BHT and Business Development teams regarding aftercare needs. Facilitate a monthly community education session on the benefits of engaging in recommended aftercare (PHP, IOP, OP, supportive housing). Maintain accurate documentation related to aftercare coordination. Develop and Grow Your Career! Position: Behavioral Health Therapist Shift: Full-Time, Monday through Friday, 8:00 am to 4:30 pm with 1 weekend every 4 weeks of on call requirements. Experience: 1. Knowledge of mental health and/or substance abuse treatment standards. 2. Emergency psychiatric and/or acute psychiatric experience preferred. Education: 1. Bachelor's degree required. 2. Master's degree or above from an accredited college or university with a major in psychology, social work, counseling, nursing or other related field preferred. Licensures/Certifications: Will help obtain certification or licensure as an addictions counselor or licensed mental health professional if not already obtained.
