Position Overview
We are seeking a detail-oriented Hospital at Home Coordinator to support our Hospital at Home program during specified hours. This remote role is essential in ensuring seamless patient intake, scheduling, and visit verification while maintaining strong communication with our clinical and operations teams.
The ideal candidate is highly organized, comfortable working in a fast-paced healthcare environment, and experienced with home health systems and coordination workflows.
Key Responsibilities
* Manage patient intake and service coordination for the Hospital at Home program
* Schedule skilled and non-skilled visits and verify completed services
* Work within Kinnser, AlayaCare, and Smartsheets to support workflow and tracking
* Communicate with clinicians and internal teams to ensure continuity of care
* Support HR by following up with candidates interested in the Hospital at Home program
* Maintain accurate records and timely documentation
* Assist with operational needs during evening and weekend hours
Qualifications
Preferred Experience:
Experience in home health care, scheduling, intake, and care coordination is strongly encouraged. Successful candidates will demonstrate:
* Excellent communication and follow-up abilities
* Proficiency with healthcare software systems (Kinnser and/or AlayaCare preferred)
* Strong attention to detail, multitasking skills and organizational abilities
* Comfort working with multiple software platforms
* Ability to work independently in a remote environment
* Reliable internet connection and dedicated workspace
Preferred:
* Experience supporting hospital-at-home or acute home care programs
* Familiarity with both skilled and non-skilled home health services.
* Basic HR or recruiting coordination experience
Experience with Kinnser, AlayaCare, and Smartsheets is highly preferred.
