Pay From: $36 per hour

MUST:

Licensed as a Licensed Practical Nurse in the District of Columbia


Flexible to work Wednesdays and other days as needed

Experienced Licensed Practical Nurse with 1+ years of direct patient care experience

Recent acute care experience in medical Emergency, Urgent care, or other inpatient units or experience working with homeless population, patients with behavioral health or being treating for mental illness or substance use disorders

Experience in/passion for public health and community nursing


Handling wound care

Ability to perform well in high-pressure or stressful situations

Excellent communication and interpersonal skills

Current DC Licensed Practical Nurse license

Current BLS/CPR certification

DUTIES:

Provide direct patient care


Handle wound care

Monitor and assess patients

Obtain vital signs

Assist with medication management

Gather information for a clinical care plan,

Educate clients on medication administration and management, overall health management and condition monitoring.

Supervise the CNA, ensuring that they are fulfilling all their responsibilities over the course of the twelve-hour shift (vital signs, ADL assistance, documentation, etc).

Monitor patients closely for changes in health status

Complete wound care and other nursing


Communicate and develop plan of care with the medical provider

Complete daily or frequent nursing assessment and/or medical exam as indicated by care plan

Identify and implement strategies to support independence in health management.

Document all clinical care provided during medical respite stay

Other duties as assigned
