Job Title Care Management Support Coordinator II Position Purpose Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact for members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines. Key Responsibilities * Provide outreach to members via phone to support care plan next steps, address questions, and connect them with health plan or community resources. * Serve as frontline support for member and provider inquiries, including explaining care plan procedures and protocols. * Support member onboarding and administrative duties such as sending welcome letters, correspondence, and educational materials. * Document and maintain nonclinical member records to ensure compliance with state and regulatory requirements. * Make referrals to address Social Determinants of Health (SDOH) needs. * Perform daily checkins and communication via group IMs. * Complete daily production logs and submit by 9am the following business day. * Multitask across multiple monitors and applications, utilizing work guides and resources. * Meet performance metrics: 97% Quality (Audits), 98% Production, 0-5% Error Rate. * Perform other duties as assigned and comply with all policies and standards. Education & Experience * Required: High School diploma or GED * Preferred: LPN, LVN, or LCSW * Experience: 1-2 years of related experience; entrylevel candidates considered Additional Skills & Qualifications * Strong communication and customer service skills * Ability to work collaboratively in a team environment * Proficiency with Microsoft Office and multiple systems/applications * Strong organizational skills and attention to detail * Ability to adapt to high authorization volumes and crosstrain across functions Job Type & LocationThis is a Contract position based out of Farmers Branch, TX. Pay and BenefitsThe pay range for this position is $22.85 - $22.85/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
