Clinical Pharmacist - Oncology

Location: Newark, DE
Position Type: FullTime, Onsite

Medical Oncology Hematology Consultants, Delaware's largest cancer treatment group, is seeking a Clinical Pharmacist to join our oncology care team in Newark, Delaware. This onsite role supports safe, effective, and evidencebased medication therapy management for oncology patients and works collaboratively with physicians, nurses, and other healthcare professionals to optimize patient outcomes.

The practice is affiliated with the US Oncology Network, providing access to national clinical guidelines while delivering care in a community-based setting.


Responsibilities

Essential Duties and Responsibilities:

  • Perform initial patient evaluations and work-ups related to disease status and anticipated prescribed therapy.

  • Collect, analyze, interpret, and evaluate data relevant to patient therapy. Obtain histories and other patient data to optimize drug therapy and nutritional support management, maximizing comprehension of medication administration and side effects.

  • Establish therapeutic objectives and monitor patients for desired therapeutic outcomes. Evaluate the significance of signs and symptoms relative to disease processes and recommend modifications in drug therapy.

  • Detect and evaluate the clinical importance of adverse reactions and complications associated with drug therapy. Provide counseling and answer questions from patients, family, and healthcare professionals concerning drug therapy.

  • Maintain active involvement with clinicians in the development and implementation of a standard drug order format and ordering process.

  • Maintain thorough and appropriate documentation in the patient clinical chart of physician orders and communication with other healthcare professionals.

  • Coordinate with physician staff and service providers in planning and implementing patient care plans to ensure safe and timely delivery of drug therapy.

  • Review and perform QA checks on all new and repeat medication orders. May prepare or supervise the preparation and dispensing of drug therapy according to Company policies and procedures.

  • Present training and in-service programs as necessary.

  • Provide educational initiatives for patients, caregivers, and healthcare team members.

  • Participate in policy development, advocacy, and institutional committees.

  • Review and recommend treatment plans in the EMR.

  • May supervise the activity of Pharmacy Technicians.


Qualifications

Minimum Qualifications
  • Bachelor's degree or PharmD from an accredited school of pharmacy
  • Current, active pharmacist license in good standing
  • Minimum of 2 years of experience as a registered pharmacist
  • Oncology experience preferred
Knowledge, Skills, and Abilities
  • Strong clinical judgment and attention to detail
  • Ability to work effectively in a fastpaced clinical environment
  • Excellent communication and collaboration skills
  • Knowledge of oncology pharmacotherapy, medication safety, and regulatory requirements
  • Commitment to patient-centered care and clinical excellence
Physical and Work Environment Requirements
  • Ability to lift up to 40 pounds
  • Vision and manual dexterity sufficient for sterile compounding and clinical review
  • Regular exposure to chemotherapy agents and pharmacy preparation environments
  • Frequent standing, walking, and patient interaction
