RN Heart Failure Program Manager

Location Royal Oak and will also be supporting at Dearborn and Troy Hospital.

Scope of Work:
Manages the quality and operations of the heart failure and VAD program in collaboration with heart failure administrative, medical and surgical leadership. Manages daily program operations and outpatient clinic operations in conjunction with practice manager, as well as collaborates with other departments within the hospital(s). Partners with leadership to provide support and guidance to heart failure program/clinic team members, which may include clinical nurses, social workers, financial counselors, and other job titles. Develops and oversees outreach clinics at various sites in the East region and participates in heart failure related activities at any CHE campus. Oversees quality/data outcome management; regulatory required monitoring and reporting.
Essential Functions
  • In conjunction with the Director of Clinical operations, medical and surgical leadership, provides high quality management of the heart failure and VAD program.
  • Participates in multidisciplinary activities including steering committee facilitation, heart failure LOS, readmission, mortality reduction, complication and safety initiatives, at all CHE campuses. Assists clinic operations in collaboration with practice manager.
  • In conjunction with the leadership team, coordinates activities of the heart failure and VAD program including resource and supply management, adherence to policies/procedures, revenue integrity, and other regulatory requirements. Assists with developing mechanisms to obtain, monitor, and disseminate information on patient satisfaction, customer service, productivity, and employee satisfaction.
  • In collaboration with the heart failure team, implements and monitors the quality improvement program and ensures compliance with regulatory agency standards.
  • Participates in new program staff interviews. Establishes and maintains open communication to enhance cohesive team environment.
Qualifications
  • Required Bachelor's Degree Nursing
  • Preferred Master's Degree
  • 3 years of relevant experience relevant experience planning, developing, and evaluating policies and programs Required
  • clinical experience Required
  • experience in heart failure cardiology Preferred
  • Registered Nurse (RN) - State of Michigan Upon Hire required
