The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: RWJBH is seeking a Clinical Care Coordinator, Registered Nurse for our Radiation Oncology Department at the New Brunswick campus. The Clinical Care Coordinator ensures continuity of care through clear communication on patient status with the inpatient unit staff and department staff as appropriate. This includes proactively managing patients who are in the department from an inpatient unit or patients to be admitted to the inpatient unit. Assists with the coordination of patient care services from the inpatient setting to the ambulatory setting or vice versa by communicating with the floor nurses as needed. Works with the office coordinator to ensure that all relevant clinical information is obtained, reviewed and available in Radiation Oncology ARIA prior to the consult and follow up visit. Works with physician to ensure follow up orders are written and carried out proactively and ensures all orders are completed and signed off according to RWJ Hospital and Department of Radiation Oncology Policy.
