Location: 12801 Westlinks Drive,Fort Myers FL 33913 (Main Home Health Office)

Department: Quality Improve - Education - Home Health

Work Type: Full Time (80 hours bi-weekly)

Shift: Days, 8:00 AM to 4:30 PM, up to 6:30 PM, Monday - Friday Work Week. (5 days/8 hour and 4 days/10 hour work weeks alternating)

Minimum to Midpoint Pay Rate:$30.27 - $40.86 / hour

After 90 days and training period: This position works from home 80% and in the office 20% (changes may take place based on business needs. Percentages are approximate)

Oasis and Home Health Coding Certifications are required. (see below)

Case management experience required. Home Health Case Management experience preferred.

Summary

Office based position under direction of the Director Home Health Quality and Compliance. Responsible for analysis of SOC, Recertification and Resumption (and Other Follow-up OASIS) OASIS and accompanying documentation, management of proper utilization of resources to ensure positive patient outcomes and optimal financial outcomes.
Teamwork, flexibility, and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering team member PTO, and other requested office tasks.
I understand the expectation is that I, as a Resource Specialist, will be certified in Home Care Coding within 12 months of hire; the expectation is to maintain this certification. See also LMHS Tuition Reimbursement Policy and Procedure.

Education: Associate degree in nursing required. Bachelors degree preferred. Bachelors degree in physical therapy required. Masters degree preferred.

Experience: Two (2) years of Case Management preferred. Two (2) years of Home Health case manager experience preferred.

Additional Requirements: Qualified individuals will successfully complete the competency assessment.

Certification: OASIS Certification required. Home Health Coding Certification required; must obtain within 12 months of hire.

License:Registered Nurse or Physical Therapist License in the State of Florida required. Valid Florida drivers license within 30 days of employment.
